I would add a bar onthe edges for a 12 inch pattern. You will know the scope of work and price before the first hammer swings. In his opinion and the way roads and bridges are built the 3k concrete is more than enough with no rebar or mesh in the middle.

A bridge here is only made with 4500 psi concrete and thats for any kind of weight or abuse so take it for what its worth.I welded up some scrap angle into 90 deg. The slab will definitely crack on the four-square lines but you will have done all you could.
Soft cuts allow the installer to plan the event rather than have some random, angled crack which looks like a mistake. I used wire mesh and 10m(3/8") rebar on 4 ft centres. Remember … all concrete cracks …. A heavy wire , 8 gage ( American Wire Gage) is 0.123" diameter . I wish I used more as I have 1/32" or so cracks every 16ft. I have no rebar or mesh in the middle part and have had no trouble at all and it routinely has a 25K Internatioanal winch truck on it. A: I advise welded wire fabric (6x6x10x10 mesh – 6″x6″ pattern with #10 gauge wire both directions), low slump concrete which means poured pretty dry (not a lot of water mixed in), 3,000 psi concrete, air entrained (little air bubbles in the cured concrete allowing water freeze expansion to fill the air bubbles instead of expanding inside the concrete causing stress), and soft cuttings (meaning weakening the concrete with tools in a straight line inviting the concrete to crack in a predetermined, usually straight, line when it cracks.Better yet is to use both the steel rebar with the fiber mesh.
When it comes to using rebar vs. wire mesh, the short answer is: use both! between wire or rebar.
