Ruby is trapped in a basement, her legs don't work, and she can't remember who she is or how she got there.

AND it does explain the title in the film for anyone confused lol

After seeing the movie my best guess regarding the title is that it refers to the "hippopotamus" in the room: that big reveal that you'll never see coming!!! Well, first it's not supposed to be a hippo, but an elephant (granted, that title was already taken by Alan Clarke and Gus Van Sant and that's one of the only qualities I can find about "Hippopotamus", it doesn't try to compare with those previous movies, which it absolutely shouldn't) and second, the first guess you'll get about 10 minutes into the movie thinking "that can't be it, it would be too dumb and make no sense at all" is actually the correct one.

If you're into slow-burn films filled with suspense and more questions than answers, this is the film for you. With a poet’s passion for the truth, Ted hurries to debunk the miracles and save a young man from a lifetime of embarrassment. 0 out of 1 found this helpful. Yet you shouldn't misunderstand slow burning as a tactic, to cover the faults of a micro budgeted film.

The acting is pretty bad too, in that regard hippos would probably have made a better job (why choose an actor which such an annoying voice when he's talking throughout the whole movie?) 12 out of 38 found this helpful. Disgraced poet Ted Wallace (Roger Allam) is summoned to Swafford Hall, the country manor of his friends Lord and Lady Logan (Matthew Modine and Fiona Shaw), to investigate a series of unexplained miracle healings. Even though what's going on is clearly right there they're not going to tell you what it is.
